You just spike the oranges with cloves. Originally, that was a custom for the summer, because it a) smells nice, and b) keeps flies and other insects away, but because the smell of cloves is now rather associated with Christmas, cloved oranges are for Christmas decoration nowadays.

The cloves contain eugenol (2-Methoxy-4-allylphenol), though, which can be used as an anaesthetic (hence the custom of chewing cloves when you have a toothache) and insecticide (hence the custom of cloved oranges against flies) and is just the tiniest bit intoxicating when you keep absorbing it through your fingers like you do when you spike three oranges with the stuff (as that takes a few hundred cloves)...
